A team asks two agents to review a design and gets two green recommendations. The visual looks like independent validation. If both saw the same incomplete brief, the second approval may add very little information about what the brief omitted.

Imagine a grid of invariants: normal traffic is green, peak traffic is blank, recovery is blank. Two summaries color the normal row twice. Neither can fill the blank cells without new data or a real test.

Place each reviewer claim beside the source it used. Mark unobserved conditions as unknown. Assign a different reviewer the task of finding a reversing case from incidents, customer constraints and the operating environment, then run the cheapest credible probe.

Before accepting multiagent consensus, require a coverage map, at least one independent source of counterevidence, and a test of the decisive unknown. Do this because duplicate approval does not fill a missing fact.
